Ozone Therapy and Inflammation: What Patients Should Know

Inflammation is part of the body’s normal defense system. When you are injured or fighting an infection, inflammatory signals help the body respond and begin healing.

But inflammation can become a problem when it is persistent or poorly regulated. Chronic inflammation is associated with a wide range of health conditions and is one reason researchers continue to study therapies that may influence inflammatory pathways.

Where does ozone therapy fit?

Medical ozone therapy uses a controlled mixture of oxygen and ozone. Researchers believe its effects may come from the way ozone briefly interacts with blood and other biological fluids, creating signaling molecules that can affect oxidative balance, immune activity and inflammatory pathways. This is very different from breathing ozone in the air. Inhaled ozone is harmful to the lungs and can itself cause inflammation and respiratory injury. PubMed

Why would an oxidant potentially reduce inflammation?

It sounds contradictory.

Ozone is a strong oxidant. But under carefully controlled conditions, researchers are studying whether a small, temporary oxidative stimulus may activate the body’s own protective systems.

This idea is often described as hormesis: a mild stressor triggers an adaptive response.

Experimental studies suggest ozone-related signaling may influence pathways such as Nrf2, which is involved in antioxidant defenses, and NF-κB, which helps regulate inflammation. Most of this mechanistic evidence, however, still comes from laboratory and animal research rather than large human clinical trials. PubMed Central (PMC)

What does the clinical evidence show?

Research in humans has investigated ozone therapy for conditions including musculoskeletal pain, wound healing, diabetic foot ulcers and some dental conditions.

Some studies have reported benefits, but the evidence is not yet consistent across diseases or treatment methods.

A 2026 review found that ozone therapy has biologically plausible anti-inflammatory and antioxidant mechanisms, but that many clinical studies remain small, use different treatment protocols and lack the quality needed to establish effectiveness conclusively. PubMed

A separate 2026 umbrella review of randomized-trial meta-analyses also found that results vary considerably by condition and that the certainty of the available evidence remains limited. PubMed

An evolving field

Ozone therapy is an active area of research, particularly because of its potential effects on oxidative signaling, inflammatory pathways and immune regulation.

The science is interesting, but it is also still developing.

For patients, the important distinction is between a promising biological mechanism and a treatment whose benefit has already been firmly demonstrated. At present, those are not always the same thing.
