New Patients will be required to leave a $50.00 deposit.
All payments made with a credit or debit card will incur a 3% processing fee at the time of payment.
Complete the Ozone Informed Consent Form ONLY if receiving Ozone Therapy including Chelation/Prolozone.
All therapies are MD ordered and supervised and RN administered in the state of NY.
Isadora Guggenheim doesn’t charge a consulting fee for any client who signs up for a minimum of 3 months of prescribed supplements.
Phone consultations – For simple questions that require a few minutes discussion there is no charge. For more complex decision-making calls, the relevant rate applies.
Payment for physician or other healthcare practitioner services, dispensary cares, diagnostic tests, and all other items is required at the time of services rendered unless other arrangements have been made with the business manager.
If pressing financial hardship should interfere with your ability to pay for services, please discuss this with our business manager before your initial visit. Every effort will be made to arrange a flexible payment schedule to meet your needs. Payment can be made by cash, check or credit card.
Cancellations for all appointments must be made within 48 hours of the scheduled appointment. Messages can be left at (845) 358-8385. If the appointment is not cancelled within this time period then you will be charged half of the fee.
